Job summary

Healogics LLC is seeking a Licensed Practical Nurse (LPN) for our Wound Care Center. You will provide wound-related and basic nursing care under RN or provider direction, following hospital protocols and Healogics policies.

Responsibilities include assisting in the vein clinic after training, supporting case management where allowed, gathering patient information, and ensuring quality of care through clear communication with the team.

Qualifications

• High School Diploma or GED required.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN or LVN license in the state of practice throughout employment.
• Two or more years’ experience as LPN/LVN, including one+ year in SNF or LTACH.
• Wound care experience preferred.

Responsibilities

• Under RN/Provider direction, provide wound-related patient care per BLS and hospital protocols.
• Assist in vein clinic after Healogics Vein Clinic training; may assist with intake and discharge.
• May assist with case management under RN guidance; adhere to state regulations.
• Gather patient information and necessary equipment for RN/provider.
• Report quality of care issues to Clinical Nurse Manager; communicate with team about interventions.
• Perform hyperbaric oxygen therapy duties after Healogics training.
• May serve as Documentation Assistant (scribe) per policy.

Job description

Licensed Practical Nurse (LPN) in the Wound Care Center provides wound‑related and basic nursing care to patients under RN or provider direction, following Healogics policies and standard protocols.

Essential Functions / Responsibilities

• Under the direction of the RN/Provider, provides wound‑related patient care according to Basic Life Support (BLS) and approved hospital protocols, policies & procedures.
• Assist in vein clinic as assigned after Healogics Vein Clinic training completed; may assist with intake and discharge.
• May provide assistance with case management under the direction of RN, in accordance with state regulations.
• Gathers patient information.
• Gathers necessary equipment and supplies for the RN or provider.
• Reports quality of care issues to Clinical Nurse Manager.
• Communicates as needed with all team members regarding therapeutic interventions to ensure quality of care.
• Performs hyperbaric oxygen therapy related duties, as needed, after completing the required Healogics hyperbaric training.
• May perform hyperbaric safety director duties after completing the required Healogics hyperbaric safety director training.
• May function as a Documentation Assistant (scribe) in accordance with Healogics policy.
• Performs other duties as delegated.

Required Education, Experience and Credentials

• High School Diploma or General Education Development (GED).
• Graduate from an accredited practical nursing program/state approved educational program.
• Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of practice – to maintain throughout the duration of employment.
• Two (2) or more years’ experience as LPN/LVN, to include one (1) or more years in SNF or LTACH setting.
• Wound care experience preferred.

Compensation

The hourly rate for this position generally ranges between $23.30 and $30.69 per hour, based on potential employee qualifications: education, experience, geography as well as operational needs and other considerations permitted by law.
