Job summary

Cortlandt Healthcare is seeking licensed practical nurses (LPNs) to join our clinical team at Cortlandt Manor, NY. We offer full-time and part-time opportunities with competitive pay and a supportive environment designed to enhance resident care.

In this role you will provide high-quality nursing care, administer medications, monitor residents, document accurately, and work closely with RNs, CNAs, and physicians to meet each resident's needs. A NY LPN license is required.

Qualifications

• Current New York State LPN license.
• Previous nursing experience preferred; skilled nursing experience is a plus.
• Strong clinical, communication, and organizational skills.
• Compassionate and resident-focused approach.
• Ability to work effectively as part of a team.

Responsibilities

• Provide high-quality nursing care to residents in accordance with individualized care plans.
• Administer medications and treatments as ordered.
• Monitor residents’ conditions and report changes to the RN and medical team.
• Complete accurate and timely clinical documentation.
• Work closely with CNAs, RNs, physicians, and other members of the interdisciplinary team.
• Assist with assessments, care planning, and resident needs.
• Promote resident safety, dignity, comfort, and quality of life.
• Communicate effectively with residents and their families.
