Job summary

The Hamptons Center for Rehabilitation & Nursing in Southampton, NY is seeking dedicated Licensed Practical Nurses (LPNs) to join a compassionate team. The role focuses on safe medication administration, wound care, and resident advocacy within a fast-paced SNF environment.

Full-time schedules available (7:00 AM–7:40 PM or 7:00 PM–7:40 AM). Competitive hourly pay and comprehensive benefits are provided to support residents and staff alike.

Qualifications

• Current New York LPN license is required.
• SNF experience preferred.
• Strong clinical and documentation skills are expected.

Responsibilities

• Administer medications and treatments per physician orders.
• Provide wound care and nursing procedures.
• Monitor residents and report changes promptly.
• Document care, observations, and interventions accurately.
• Order medications, supplies, and equipment as needed.
• Collaborate with RNs, CNAs, physicians, and the team.
• Maintain resident safety, dignity, and comfort.
• Adhere to HIPAA, infection control, and facility policies.
• Communicate effectively with residents, families, and healthcare professionals.

• Document care, observations, and interventions accurately
• Order medications, supplies, and equipment as needed
• Collaborate with RNs, CNAs, physicians, and the interdisciplinary team
• Maintain resident safety, dignity, and comfort
• Follow HIPAA, infection control, and facility policies
• Communicate effectively with residents, families, and healthcare professionals

Qualifications

• Valid New York State LPN license
• Skilled Nursing Facility (SNF) experience preferred
