Job summary

The Hamptons Center for Rehabilitation & Nursing is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team in Southampton, NY. This full-time role offers day and night shift options and pay in the $35.20–$36.20 per hour range.

Responsibilities include administering medications, wound care, resident monitoring, and collaborating with an interdisciplinary team to ensure high-quality, person-centered care for residents.

Qualifications

• Valid New York State LPN license is required.
• SNF experience preferred.
• Strong clinical and documentation skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Administer medications and treatments per orders.
• Provide wound care and nursing procedures.
• Monitor residents and report changes promptly.
• Document care and observations accurately.
• Collaborate with RNs, CNAs, physicians, and the team.
• Maintain resident safety, dignity, and comfort.
• Follow HIPAA and infection-control policies.
• Communicate with residents, families and health professionals.

• Provide wound care and perform nursing procedures
• Monitor residents and promptly report changes in condition
• Document care, observations, and interventions accuratelyOrder medications, supplies, and equipment as needed
• Collaborate with RNs, CNAs, physicians, and the interdisciplinary team
• Maintain resident safety, dignity, and comfort
• Follow HIPAA, infection control, and facility policies
• Communicate effectively with residents, families, and healthcare professionals
