About the Role

As an LPN at Autumnwood Care Center, you will be responsible for:

• Providing direct patient care to our residents, including administering medications, wound care, and vital sign monitoring

• Developing and implementing individualized care plans in collaboration with other members of the healthcare team

• Maintaining accurate and up-to-date records of patient care and progress

• Participating in quality improvement initiatives to enhance patient outcomes and satisfaction

• Collaborating with other departments, such as rehabilitation and social services, to ensure comprehensive care

Responsibilities

• Provide direct patient care, including:

  • Administering medications as prescribed

  • Conducting vital sign monitoring and reporting any abnormalities

  • Providing wound care and other treatments as needed

  • Assisting with activities of daily living (ADLs) such as bathing, dressing, and grooming

• Develop and implement individualized care plans in collaboration with other healthcare professionals

• Maintain accurate and up-to-date records of patient care and progress

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Collaborate with other departments to ensure comprehensive care

• Perform other duties as assigned by the nurse manager or other supervisors
