As an LPN at Autumnwood Care Center, you will be responsible for:
Providing direct patient care to our residents, including administering medications, wound care, and vital sign monitoring
Developing and implementing individualized care plans in collaboration with other members of the healthcare team
Maintaining accurate and up-to-date records of patient care and progress
Participating in quality improvement initiatives to enhance patient outcomes and satisfaction
Collaborating with other departments, such as rehabilitation and social services, to ensure comprehensive care
Provide direct patient care, including:
Administering medications as prescribed
Conducting vital sign monitoring and reporting any abnormalities
Providing wound care and other treatments as needed
Assisting with activities of daily living (ADLs) such as bathing, dressing, and grooming
Develop and implement individualized care plans in collaboration with other healthcare professionals
Maintain accurate and up-to-date records of patient care and progress
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Collaborate with other departments to ensure comprehensive care
Perform other duties as assigned by the nurse manager or other supervisors
