Millennial Healthcare in The Villages, FL is seeking a Licensed Practical Nurse (LPN) for a night shift (11p-7a) with a full-time option and part-time schedule on alternating weekends. The skilled nursing and rehabilitation facility emphasizes compassionate care, safety, and a positive work environment.

Responsibilities include nursing care per physician orders, documentation, and collaboration with the nursing leadership to ensure high-quality resident care.

Qualifications

• Licensed Practical Nurse license active in Florida.
• Minimum 1 year of Skilled Nursing experience preferred.
• Current/active CPR certification.
• Excellent technical, assessment, and documentation skills.
• Excellent English communication skills.
• Professionalism, confidentiality, and ability to work within a team.

Responsibilities

• Assess and document resident progress in the medical record.
• Monitor turning and positioning of residents.
• Obtain changes in physician’s orders as indicated by resident’s progress.
• Communicate pertinent information about resident condition to charge nurses and the Director of Nursing.
• Maintain clean, orderly treatment cart and restock supplies.
• Follow Infection Control policies and procedures.
• Administer medications and treatments as ordered.
• Document pressure sore status weekly or as needed.
• Assist in inventory of medications and supplies.
• Provide resident/family education on skin care prior to discharge home.
• Participate in Quality Improvement activities as assigned.

Summary of the Licensed Practical Nurse (LPN):

Provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practice and regulations and as directed by the Director of Nursing.

Essential Duties & Responsibilities of Licensed Practical Nurse (LPN):

• Assess and document resident progress in the medical record.
• Monitor turning and positioning of residents.
• Obtain changes in physician’s orders as indicated by resident’s progress or lack of progress
• Communicate pertinent information about resident condition and progress to charge nurses, supervisors and Director of Nursing, as indicated.
• Maintain clean, orderly treatment cart and restock treatment supplies.
• Follow Infection Control policies and procedures.
• Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures.
• Document pressure sore status weekly or more often as condition changes.
• Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents.
• Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home.
• Participate in Quality Improvement activities, as assigned.

Job Requirements of Licensed Practical Nurse (LPN):

• Licensed Practical Nurse current state license in Florida.
• At least one (1) year of Skilled Nursing experience preferred
• Must have a current/active CPR certification.
• Excellent technical, assessment, and documentation skills.
• Effective verbal and written English communication skills.
• Highest level of professionalism with the ability to maintain confidentiality.
• Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
• Customer service oriented with the ability to work well under pressure.
• Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
• Strong analytical and problem-solving skills.
