The Pavilion at Stow is a skilled nursing and rehabilitation center located in Stow, OH. We are seeking a Licensed Practical Nurse (LPN) to provide direct nursing care and supervise CNAs in accordance with regulations and facility policies.

This PRN position offers opportunities to support residents' daily care, manage medications, and participate in admissions, discharges, and transfers. A current LPN license, CPR certification, and IV certification if applicable are required.

Qualifications

• CPR certification required.
• Active LPN license.
• IV certification preferred if applicable.
• Prior long-term care experience preferred but not required.

Responsibilities

• Directs daily activities of CNAs in accordance with policies and regulations.
• Promotes holistic care following the nursing model.
• Ensures compliance with policies by nursing staff.
• Participates in admissions, discharges and transfers of residents.
• Assesses changes in resident status and communicates with physician/family.
• Transcribes orders and carries them out accurately.
• Prepares and administers medications per physician orders.
• Performs wound care and documents changes.
• Schedules follow-up appointments and transportation as needed.
• Provides nursing leadership on the unit.
• Performs other duties as assigned.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.

• Follows the nursing model to promote holistic care for the residents.

• Ensures that policies and procedures are complied with by nursing personnel assigned.

• Participates in the admission, discharge and transfers of residents as required.

• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.

• Transcribes physician orders to medical record and carries out orders as written.

• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.

• Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated.

• Schedules follow up appointments for residents and transportation needs as indicated.

• Provides nursing leadership to nursing personnel assigned to the unit.

• Other duties as assigned.

Qualifications

• Prior experience in long-term care preferred but not required.

• A Nursing degree from an accredited college or university.

• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.

• Current CPR certification.

• IV certification, if applicable
