Job summary

Sunnyside Care Center is seeking licensed practical nurses (LPNs) to join our nursing team in a long-term care setting. You will provide direct patient care, maintain safety and privacy, and support patients and families in a compassionate manner.

Responsibilities include monitoring patients, reporting changes to physicians, discharging patients when needed, and assisting with transportation. Previous LPN experience and long-term care exposure are preferred; work is in person in East Syracuse,

Qualifications

• Long term care: 1 year (Preferred)
• LPN (Preferred)
• Previous experience in a medical setting and/or as a Licensed Practical Nurse is required for this position.

Responsibilities

• Set up a safe environment for patients.
• Provide effective nursing care to patients.
• Provide emotional support to patients and their families.
• Report changes in patient's conditions to physician or registered nurse.
• Discharge patients and assist in transportation if needed.
• Record observations and vital signs.
• Inform physicians of patient's progress and of any problems that require their attention.
• Arrange for special services (i.e. meals) for the patient if necessary.
• Any other duties as assigned by the Director of Nursing

Sunnyside Care Center is actively looking for LPNs to join our nursing team. As a Licensed Practical Nurse, you will be responsible for providing direct patient care and support to patients.

The LPN will also assist in maintaining high standards of patient safety, privacy, care and cleanliness. Previous experience in a medical setting and/or as a Licensed Practical Nurse is required for this position.
