Job summary

Avalon Health Care Group is seeking an LPN to provide direct nursing care at the Southern Utah Veterans Home in Ivins, St. George, UT. You will work under an RN and collaborate with the healthcare team to ensure compassionate care for our veteran residents.

The role requires an active Utah LPN license and BLS certification, with 1 year of nursing experience preferred in long-term care. You will participate in care planning and education for residents and families.

Qualifications

• Current and valid LPN license in Utah.

Responsibilities

• Maintain accurate documentation in residents’ medical records.
• Respond promptly to resident needs and emergencies.
• Educate residents and families on health maintenance and disease prevention.
• Adhere to facility policies, procedures and regulatory requirements.

Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to residents at the Southern Utah Veterans Home. The LPN works under the supervision of a Registered Nurse (RN) and collaborates with the healthcare team to ensure high-quality, compassionate care for our veteran residents.

Key Responsibilities

• Administer medications and treatments as prescribed by physicians and according to facility policies.
• Monitor and document residents’ health status, including vital signs, symptoms, and changes in condition.
• Assist with the development and implementation of individualized care plans.
• Provide wound care, catheter care, and other nursing procedures as required.
• Collaborate with RNs, physicians, and other healthcare professionals to coordinate resident care.
• Maintain accurate and timely documentation in residents’ medical records.
• Respond promptly to resident needs and emergencies.
• Educate residents and their families on health maintenance and disease prevention.
• Adhere to all facility policies, procedures, and regulatory requirements.

Qualifications
