Franklin County Rehab is seeking Licensed Practical Nurses (LPNs) to join our skilled nursing team. The role emphasizes compassionate, resident-focused care and collaboration with a multidisciplinary team.

The successful candidate will provide direct nursing care, administer medications, supervise CNAs, and maintain accurate documentation while upholding safety and infection-control standards.

Qualifications

• Current, unencumbered LPN license required.

Responsibilities

• Provide direct nursing care to residents in accordance with care plans, physician orders, and facility policies.
• Administer medications and treatments safely and accurately.
• Provide wound care and other nursing treatments as assigned.
• Supervise and support CNAs and other assigned staff.
• Monitor residents' conditions and promptly report changes to the appropriate nursing team members.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Communicate effectively with residents, families, physicians, nurses, and other healthcare professionals.
• Maintain accurate and timely documentation of resident care and progress.
• Promote resident dignity, independence, safety, comfort, and quality of life.
• Follow infection control, safety, and facility policies and procedures.
• Demonstrate professionalism, compassion, teamwork, and respect in all interactions.

Franklin County Rehab is seeking highly motivated, compassionate, and dependable Licensed Practical Nurses (LPNs) to join our dedicated skilled nursing team.

As an LPN at Franklin County Rehab, you will play an important role in providing exceptional care to our residents while working alongside nurses, nursing assistants, and other members of the interdisciplinary care team.
