Collingswood Rehabilitation and Healthcare Center is seeking an LPN for full-time, part-time, or PRN opportunities. Shifts include 7am-3pm (part-time) and 11pm-7am (full-time/part-time/PRN). Compensation ranges from $31–$40 per hour, all inclusive.

The facility emphasizes compassionate, high-quality nursing care for residents. Responsibilities include administering medications, following care plans, and collaborating with the interdisciplinary team to deliver safe, person-centered care.

Qualifications

• Active LPN license in Maryland is required.
• BLS/CPR certification required.
• Ability to care for and manage multiple patients in a fast-paced setting.
• Strong clinical judgment, communication, and attention to detail.

Responsibilities

• Provide patient care per standards and plan of care.
• Follow individualized care plans with Nurse Manager and team.
• Administer medications and treatments per care plan and policies.
• Monitor residents' conditions and report changes promptly.
• Maintain accurate documentation per orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Maryland
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
