Job summary

Canterbury Rehabilitation and Healthcare Center is seeking an experienced Licensed Practical Nurse (LPN) to join our compassionate team. This full-time role supports residents with individualized care plans, medication administration, and ongoing assessments in a sub-acute and long-term care setting.

Shifts available from 7:00 AM to 3:00 PM, 3:00 PM to 11:00 PM, and 11:00 PM to 7:00 AM. Daily pay options are offered in a supportive work culture that values teamwork, safety, and professional

Qualifications

• Current, unencumbered active LPN license in Virginia.
• Valid BLS Certification (CPR) required.
• Ability to manage multiple patients in a fast-paced healthcare environment.

Responsibilities

• Provide patient care per standards and care plans.
• Follow individualized care plans by Nurse Manager and team.
• Administer medications per plan of care and facility policies.
• Monitor residents, report changes promptly to supervisor/physician.
• Maintain accurate documentation per orders and regulations.
• Support a safe, respectful, therapeutic environment for residents.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of va.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
