Shift: Day shift 6:00am - 6:30pm includes every other weekend and holiday

Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team within a correctional facility. The LPN will provide direct patient care to an underserved population while working collaboratively with physicians, registered nurses, correctional staff, and other healthcare professionals. Schedule: PM shift with holiday and weekend coverage.

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers.
• Perform patient assessments and document findings accurately and timely.
• Monitor and evaluate patient health status, reporting significant changes to the RN or provider.
• Provide wound care, chronic disease management, and routine nursing interventions.
• Assist with intake screenings, health assessments, and discharge planning.
• Maintain accurate medical records in accordance with facility policies and regulatory requirements.
• Respond appropriately to medical emergencies and urgent healthcare needs.
• Educate patients on health conditions, medications, and treatment plans.

Qualifications
