Rittenhouse Post Acute, a 19-bed Sub-Acute and Long-Term Care facility in Philadelphia, seeks an LPN for full-time night shift (7pm–7am) to provide compassionate nursing care. The role emphasizes adherence to care plans and resident well-being.

You will administer medications, monitor residents, document per orders, and collaborate with a multidisciplinary team to ensure safety and quality care. DailyPay and comprehensive benefits are available.

Qualifications

• Active LPN license in Pennsylvania.
• Current BLS CPR certification.
• Ability to manage multiple residents in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per the plan of care and physician’s treatment.
• Follow and implement individualized care plans by the Nurse Manager and interdisciplinary team.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Pennsylvania.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
