Job summary

Olde Main Post Acute is seeking a Licensed Practical Nurse (LPN) to join our compassionate team in Philadelphia. You will provide patient care per the plan of care, assist with medications, and support residents' safety and comfort in a busy facility.

Compensation ranges from $30 to $36 per hour with same-day pay. Benefits include tuition reimbursement, health/dental, 401(k) match, and employee programs; shifts include 7A-3P, 3P-11P, and 11P-7A.

Qualifications

• Current PA LPN license required.
• BLS (CPR) certification required.
• Ability to care for multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per the plan of care and physician orders.
• Follow individualized care plans and nurse manager guidance.
• Administer medications and treatments per policy.
• Monitor residents and report changes promptly to the supervising nurse.
• Maintain accurate documentation and ensure regulatory compliance.
• Support a safe, respectful, and therapeutic environment.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of PA.

• Valid BLS Certification (CPR) required.

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

• Strong clinical judgment, communication skills, and attention to detail.
