Job summary

Gifted Handz LLC seeks a Licensed Practical Nurse to create and manage individualized care plans for clients receiving in-home support in the Philadelphia area.

You will provide 1:1 patient care, collaborate with supervisors and caregivers, and use nursing judgement to monitor conditions, administer medications, and document changes. This role involves in-home care across Straustown PA and the Philadelphia area.

Qualifications

• Current LPN license in the state of employment.
• New grads welcome though prior LPN experience is preferred.
• Excellent interpersonal and client care skills.
• Valid driver’s license required.
• First aid/CPR certification required.

Responsibilities

• Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans.
• Administer medications and monitor patients for side effects or reactions.
• Document changes in patient conditions and report observations to the supervisor.
• Maintain accurate records of patient care and discuss observations with the supervisor.

Job Summary

We are seeking a Licensed Practical Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a LPN who operates with great attention to detail and a big heart. As a LPN with Gifted Handz LLC we care for those with Intellectual Disabilities in their homes and require a nurse who can collaborate, work independently, good people skills and nursing judgement and who is reliable. You will provide 1:1 care and support to your individual. I have several clients needing assistance in Straustown Pa, and Philadelphia Area
