Job summary

365 Health Services is seeking an LPN to provide skilled nursing care in patients' homes across the Philadelphia area. You will work with an interdisciplinary team to ensure high-quality, individualized care while maintaining independence in the field.

The role offers flexible scheduling, day/evening/overnight shifts, and various full-time/part-time options. Compensation is $30–$35 per hour with opportunities for overtime and professional growth within a supportive clinic network.

Qualifications

• Active LPN license (Pennsylvania and/or Compact License) required.

Position Overview

As a Home Health LPN, you will provide skilled nursing care to patients in their homes while collaborating with the interdisciplinary care team to ensure high-quality, individualized care.

You’ll work independently in the field - but never alone. Our Clinical Supervisors and operational staff are readily available to support you.
