Job summary

365 Health Services is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to provide one-on-one care in the homes of clients across Chester County. You can choose cases based on location, schedule, and care needs, with opportunities for consistent hours or additional shifts to fit your availability.

In this role you will deliver focused, individualized care to one client at a time, building relationships with clients and their families while adhering to plan of care, safety

Qualifications

• Current and active LPN license in Pennsylvania.
• Attend in-person orientation at Downingtown office.
• CPR certification is required.
• TB test within recent timeframe.
• Two forms of ID (including driver’s license).
• Ability to travel locally to client homes.

Responsibilities

• Provide one-on-one nursing care in the client’s home.
• Follow the client’s individualized plan of care.
• Monitor client condition and report changes.
• Administer medications and treatments within scope of practice.
• Assist with activities of daily living (ADLs).
• Maintain accurate documentation of care provided.

Role Description

365 Health Services is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to provide one-on-one care to individuals in the comfort of their homes throughout the Chester County area. Whether you're looking for consistent hours or additional shifts to fit around your current schedule, we have a variety of open cases available for you to choose from.

In this role, you will be providing one-on-one care to client(s) in the comfort of their own homes. You will be focusing on one individual at a time, get to know the client and their family, and provide consistent support based on their specific needs.

Pay & Benefits

$30-$40/hour

Key Responsibilities

Responsibilities will vary depending on the individual client's plan of care and may include:

• Providing one-on-one nursing care in the client's home
• Following the client's individualized plan of care
• Monitoring the client's condition and reporting changes as appropriate
• Administering medications and treatments as ordered and within scope of practice
• Assisting with activities of daily living and other care needs
• Maintaining accurate and timely documentation of care provided
• Strong communication skills with the client, family, and the 365 team
• Following agency policies, procedures, and applicable nursing standards
• Providing compassionate, attentive, and dependable care throughout each scheduled shift

Preferred Clinical Experience (Not Mandatory)

• G-tube care and feedings
• G/J tube care and feedings
• Tracheostomy care
• Ventilator care
• Seizure monitoring and precautions

Qualifications

• Current and active LPN license/certification in the state of Pennsylvania.
• Attend in-person Orientation at our Downingtown office
• Recent physical
• CPR certification
• Child Abuse Clearances
• Recent TB test
• Two forms of ID (including driver's license)
• Clinical care skills, including direct patient care, vital sign monitoring, assistance with mobility and ADLs, and basic wound or treatment support within scope of practice.
• Strong assessment, observation, and documentation abilities to recognize changes in condition and accurately record care provided.
• Effective communication and interpersonal skills to collaborate with consumers, families, and multidisciplinary team members.
• Knowledge of home care best practices, infection control, safety protocols, and relevant state and federal regulations.
• Reliability, professionalism, and the ability to work independently in the field while following established care plans and agency policies.
• Basic computer or electronic documentation skills and ability to learn agency systems.
• Reliable transportation and ability to travel locally to consumer homes as needed.
• Prior home health, home care, or long-term care (preferred)
