  9. Is responsible for own actions and for seeking patient information necessary to function effectively in assigned health setting. Maintains personal competency.
  10. Assists in assessment of patient needs, implements and documents nursing care under the direction of the RN.
  11. Observes the patient's condition and reports changes of condition and the need for nursing measures beyond his/her knowledge.
  12. Is an active member of the health care team assisting the Physician and/or RN in performing specialized procedures by preparing equipment and assisting in treatments while utilizing aseptic techniques.
  13. Is capable of writing legible, accurate clinical and progress notes regarding the patient's care and condition.
  14. Will assist the patient in personal care along with basic nursing care.
  15. Assists the patient's family and other health care team members in providing continuity of care and meeting the emotional needs of the patient and immediate family.
  16. Continually updates professional knowledge and skills through attendance and participation in continuing education and in-service classes.
  17. Performs all skills, treatments and procedures completely and within the regulations and licensing laws of the State Nurse Practice Acts, ACHC's policies and procedures and other applicable local/federal laws.
Education

High School Diploma or equivalent

Professional Experience
  • Current LPN license within the State of New Jersey
  • 1 year of Community Health Nursing
