Job summary

Aviata at The Palms in Palm Harbor, Florida, is seeking a dedicated Licensed Practical Nurse (LPN) to provide direct patient care to residents, supporting daily living activities under supervision of a professional nurse.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• You must be qualified, compassionate, and dedicated to a job well done.

Responsibilities

• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services.
• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment.
• Other duties as assigned.

As an LPN at our health and rehabilitation center, you will provide direct patient care to our residents, assisting them in activities of daily living under the direction and supervision of a professional nurse.
