Job summary

Ascend Hospice in Overland Park, KS is hiring a Licensed Practical Nurse to provide compassionate patient care Monday through Friday, 8:30am–5:00pm. You will work closely with the Hospice Nurse, attending physicians, and an interdisciplinary team to manage patient needs and symptom control in a home care setting.

Applicants must hold an active LPN license in Kansas and have at least one year of clinical experience; LTC/home care/hospice experience preferred.

Qualifications

• Strong verbal and written communication skills.
• Able to work as part of an interdisciplinary team.
• Current LPN license in the state of operation.
• Experience in a hospice, long-term care, or home care setting preferred.

Responsibilities

• Observe and document patient and family needs over time.
• Consult with Hospice Nurse on patient status and symptom management.
• Provide care as prescribed by physician and supervised by Hospice Nurse.
• Monitor and document patient/family input and responses to care.
• Maintain continuity of patient care through communication with families and health professionals.
• Participate in interdisciplinary team meetings and related activities.
• Ensure sufficient home care supplies for patient management.
• Assist with clinical documentation and billing as needed.
• Aid in orientation of new hospice aides as assigned.

Now Hiring - Licensed Practical Nurse - Overland Park, KS Area

Hours: Monday-Friday 8:30a-5p

TheLicensed Practical Nursewill be responsible for, but are not limited to:

1. Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis
2. Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels
3. Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team
4. Monitors and documents patient/family input and response to care
5. Maintains continuity of patient care through ongoing communication with families and other health care professionals
6. Participates in Interdisciplinary Team Meetings and all other meetings as assigned
7. Maintains sufficient and appropriate home care supplies for adequate patient care management
8. Collection of documentation and paperwork as requested by the Hospice Nurse.
9. Assistance with clinical billing as deemed necessary
10. Participates in orientation of new hospice aides as assigned

Education:

Associate's Degree in a related field

Professional Experience:

• Hold a Valid and current LPN license within the state of operations
• 1 year of clinical experience
• Long Term Care/Home Care/Hospice experience preferred
• Strong verbal and written communication skills.
• Able to work as part of an inter-disciplinary team
