Job summary

Interim HealthCare Inc. is seeking an LPN to provide 1:1, patient-centered home health care in Florida. You will assess patients, monitor vitals, document progress and communicate changes to the RN supervisor.

Qualifications

• Graduate of an accredited practical nursing program and active LPN license in Florida.

• Clinical proficiency in nursing, strong communication skills and compassion.

Responsibilities

• Provide fundamental home-based care to patients with chronic illness or disease.
• Take patient vitals, do assessments, document progress and report changes to RN supervisor.
• Educate family on the patient’s plan of care, medication and home safety.

As a Home Health Licensed Practical Nurse, a big-picture view of your job responsibilities includes:
