Job summary

Interim HealthCare Inc. is seeking an Licensed Practical Nurse (LPN) to provide in-home patient care across our community. The role emphasizes delivering compassionate, high-quality nursing services with respect for patient dignity in their homes.

We offer flexible scheduling (FT/PT/PRN), competitive pay with benefits, and a daily pay option to support work-life balance. You'll work closely with a care team to monitor progress and adjust plans as needed.

Qualifications

• Current unrestricted LPN license to practice in Ohio.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations.
• Practical trach and/or ventilator experience preferred, not required.

Responsibilities

• Provide in-home direct patient care according to the prescribed plan of care (and nursing scope of practice).
• Assess and document patient conditions; report changes as needed (vitals, labs, etc.).
• Administer medications, insulin, and IV fluids as documented.
• Inspect and care for wounds, change dressings, assist with personal hygiene.
• Collaborate with providers, aides, and allied health professionals to ensure patient outcomes.

As aLicensed Practical Nurses (LPN)you will:

• Providein-homedirect patient care according to theprovider'sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery
• Assessand document patient conditions and reportchangeas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV/fluids,documenting thoroughly
• Inspect and care for wounds, changing dressings and assisting with personal hygiene
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes

To qualify for a Licensed Practical Nurse (LPN) position with us, you will need:

• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
