Key Responsibilities:

• Conduct comprehensive client assessmentsto create personalized care plans.

• Administer medications, treatments, and nursing proceduresas required.

• Monitor clients' health status(vital signs, progress) and adjust care plans as needed.

• Educate clients & familieson disease management, medication use, and home safety.

• Performadvanced nursing proceduresincluding wound care, IV therapy, and catheter care.

• Document all care providedin compliance with regulatory guidelines.

• Ensure compliancewith state and federal healthcare regulations.

• Provideemotional supportand guidance to clients and their families.

• Stay up-to-date withnursing best practicesand participate in professional development.

What We’re Looking For:

• Active LPN Licensein the state of New York.

• 1-2 years of nursing experience(home health, geriatrics, or similar preferred).

• Strong clinical skillsand ability to perform nursing procedures independently.

• Excellent communication skillsand the ability to build rapport with clients and families.

• Self-motivated and organized, with the ability to work independently in a home care setting.

• Reliable transportationand or a valid driver’s license.

• CPR and BLS certification(required).

• Experience withelectronic health records (EHR)systems (preferred).
