Responsibilities

Position Summary

The Federally Qualified Health Center (FQHC) is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our community-based healthcare team in Brooklyn. The LPN will provide direct patient care, support providers and registered nurses, and contribute to high-quality, culturally responsive services for patients across the lifespan.

This position is ideal for an LPN who is committed to serving diverse and underserved communities and who thrives in a collaborative, fast-paced clinical environment.

Essential Responsibilities
  • Provide compassionate, patient-centered nursing care in accordance with established policies, procedures, and scope of practice.

  • Obtain and document patient vital signs, medical histories, medication information, and presenting concerns.

  • Prepare patients for examinations, procedures, and treatments.

  • Assist physicians, nurse practitioners, physician assistants, and registered nurses during clinical visits.

  • Administer medications, immunizations, injections, and treatments as ordered and within authorized scope of practice.

  • Perform wound care, dressing changes, specimen collection, point-of-care testing, and other clinical procedures.

  • Monitor patients for changes in condition and promptly communicate concerns to the appropriate provider or nurse.

  • Provide patient education regarding medications, preventive care, treatment plans, chronic disease management, and follow-up instructions.

  • Support care coordination, referrals, health screenings, and outreach activities.

  • Assist with preventive health initiatives, including immunizations, screenings, and wellness education.

  • Maintain accurate, timely, and confidential documentation in the electronic health record.

  • Follow infection prevention, safety, medication administration, and emergency response protocols.

  • Participate in quality improvement, clinical audits, patient safety initiatives, and FQHC performance-improvement activities.

  • Help maintain examination rooms, clinical supplies, equipment, and medication areas.

  • Communicate respectfully and effectively with patients, families, coworkers, and community partners.

  • Support a welcoming, inclusive, and trauma-informed environment for patients from diverse cultural, linguistic, and socioeconomic backgrounds.

  • Perform other duties as assigned.

Required Qualifications
  • Current and unrestricted New York State Licensed Practical Nurse license.

  • Graduation from an accredited practical nursing program.

  • Ability to provide safe, effective nursing care within the LPN scope of practice.

  • Strong clinical assessment, organization, documentation, and communication skills.

  • Basic computer proficiency and familiarity with electronic medical records.

  • Commitment to working with underserved and culturally diverse patient populations.

  • Ability to maintain patient confidentiality and comply with applicable healthcare privacy requirements.

Preferred Qualifications
  • One or more years of LPN experience in a primary care, community health center, outpatient clinic, ambulatory care, or similar setting.

  • Experience working in an FQHC or other safety-net healthcare organization.

  • Experience with chronic disease management, immunizations, wound care, women's health, pediatrics, or behavioral health integration.

  • Familiarity with quality measures, population health, care coordination, and electronic health record systems.
