Job description

As a member of the Hospice team, the LPN works primarily in a support role by performing tasks within the framework of case findings, health teaching, health counseling, and supportive care under the supervision of a registered nurse, carrying out the nursing process. The LPN works under the direct supervision of the RN or licensed physician and interacts with patients, spouses, significant others, family members, physicians, and team members, in line with the mission of Hospice.

ESSENTIAL FUNCTIONS:

  1. Provides services within the boundaries of the State Practice Act and responsibilities listed in the Department of Patient Services.
  2. Supports the team by providing input and updates on patient condition, supporting decisions as outlined in the hospice plan of care, under the guidance of the primary RN Case Manager.
  3. Observes and collects data through interviews and physical assessments to identify needs or problems, and applies appropriate standards.
  4. Provides anticipatory guidance and support to patients and families, as per the interdisciplinary plan of care and directed by the primary hospice RN Case Manager, Social Worker, Pastoral Care, and/or Coordinator of Patient Care.
  5. Records monitored information such as intake/output and vital signs.
  6. Participates in patient and family instruction, documenting education provided and patient understanding.
  7. Communicates daily updates on each patient to the case manager.
  8. Performs phlebotomy as needed.
  9. Assists with Activities of Daily Living (ADLs) when scheduled or necessary.
  10. Maintains hourly documentation on the Continuous Care Interdisciplinary Progress Note, following hospice documentation policies.
  11. Immediately notifies hospice in the event of patient death during a shift; the primary RN or On-Call RN attends the death.

WORK ENVIRONMENT/PHYSICAL DEMANDS:

The physical demands of this role include:

  • Working in diverse home environments.
  • Travel to patient homes using reliable transportation with a valid driver’s license.
  • Availability for all shifts with a monthly on-call weekend rotation.
  • Potential exposure to unsafe environments.
  • Willingness to work in any county as needed.
  • Ability to carry up to twenty-five pounds of equipment or supplies.

PREFERRED EDUCATION & EXPERIENCE:

  • Licensed as a Practical Nurse in New York State.
  • At least two years of nursing experience, preferably in home care or hospice settings.
  • Opportunity to attain Hospice & Palliative Licensed Nurse Certification after 500 hours or one year of employment.

Hours: Tuesday-Saturday, 8:30 am - 4:30 pm

Pay: $27-29 per hour plus mileage reimbursement!

FLSA: Non-Exempt
