Exceptional Living Centers’ Charlestown Place at New Albany, IN is seeking a compassionate Licensed Practical Nurse (LPN) to deliver skilled nursing care in our community. This position offers full-time or part-time hours with a $5,000 sign-on bonus for eligible applicants.

You will administer medications, document care plans, and collaborate with physicians and the care team to ensure high-quality, resident-centered care while adhering to regulatory standards.

Qualifications

• Active LPN license in Indiana or multi-state license.
• Experience in skilled nursing, long-term care, or healthcare preferred.
• Strong communication, organizational, and teamwork skills.
• Commitment to resident-centered care.

Responsibilities

• Deliver skilled nursing care to residents in a Skilled Nursing setting.
• Administer medications and treatments in accordance with care plans.
• Complete accurate and timely clinical documentation.
• Observe residents for changes in condition and report updates to the care team.
• Partner with physicians, leadership, and interdisciplinary team members.
• Ensure compliance with regulatory, safety, and quality standards.
