Job summary

Signature HealthCARE of Monteagle Rehab & Wellness Center is seeking an LPN to provide direct nursing care, including medication administration, wound care, and ADL assistance, in a 150-bed skilled facility.

This role requires an active Tennessee LPN/LVN license, at least one year of clinical nursing experience (long-term care preferred), and supervisory ability. Familiarity with EHR systems and MS Office is a plus. Join a team dedicated to person-directed care and quality outcomes.

Qualifications

• Active LPN/LVN license in Tennessee.
• Minimum one year of clinical nursing experience, preferably in long-term care.
• Supervisory or charge nurse experience preferred.
• Familiarity with electronic health records (EHR).
• Proficient in documenting and medication administration.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure dignity, safety and comfort.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents’ status and report changes to physicians and families.
• Develop and review individualized care plans with the team.
• Document accurately per facility policies and procedures.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.

How You Will Make a Difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What You Need To Make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
