Interim HealthCare of the Twin Cities is seeking a Home Care Licensed Practical Nurse (LPN) to visit patients in their homes, coordinating care, managing medications, and providing skilled nursing services. The role emphasizes autonomy, field work, and collaboration within an interdisciplinary team.

Applicants should hold an active MN LPN license, have CPR certification, and be able to travel within the office's geographic area.

Qualifications

• Active MN LPN license in good standing.
• Minimum 2 years LPN experience preferred.
• Valid driver’s license and reliable transportation; proof of auto insurance.
• CPR certification required.

Responsibilities

• Participate in ongoing interdisciplinary assessment of the patient.
• Provide services as ordered by the physician per the care plan.
• Recognize and report changes in patient condition to the RN.
• Contribute to development and evaluation of the plan of care with patient and caregiver(s).
• Counsel and educate patient and family on healthcare needs.
• Complete timely and accurate clinical notes addressing patient progress.

Home Care Licensed Practical Nurse (LPN)

Hours

Full‑time | Days, Monday - Friday | Work approximately one weekend every 4‑6 weeks and 2 holidays per year. The weekend and holiday rotation is dependent upon staffing levels.

Territory

This position will work in the field helping patients in their home setting. Our office is in Roseville, MN and the general territory for this position would be the southwest metro.

A few must‑has for this position

• Current license as an Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
• 2 years experience as an LPN is preferred.
• Able to travel within geographic area serviced by the office. Valid driver’s license, reliable transportation, and proof of auto insurance are required.

• Participate in ongoing interdisciplinary assessment of the patient.

• Recognize and report changes in the patient’s condition to the RN.
• Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Counsel and educate the patient and family in meeting healthcare related needs.
• Complete timely and accurate clinical notes including addressing patient’s progress.
