Job summary

Palmetto Subacute Care Center in Florida is seeking a Licensed Practical Nurse (LPN) to join our compassionate team. This role delivers skilled nursing care to residents requiring long-term or rehabilitative services, under supervision and according to the Nurse Practice Act and our policies.

You will assess residents, implement and evaluate care plans, and collaborate with a multidisciplinary team. Qualified candidates hold a Florida LPN license, CPR certification, and preferably 1 year in

Qualifications

• Current/valid Florida LPN license.
• Previous experience in long-term care preferred.
• Current CPR card.
• Ability to work independently or in a group.
• Effectively communicate with others.
• A minimum of one year nursing experience in a long-term or acute care setting preferred.

Responsibilities

• Delivers nursing care to patients/residents in long-term or rehabilitative care.
• Assesses patients/residents and reports pertinent information related to care.
• Implements the patient/resident plan of care and evaluates responses.
• Directs and supervises care given by other nursing personnel.
• Maintains knowledge of documentation requirements.
• Maintains knowledge of equipment setup, maintenance and use.
• Maintains confidentiality and patient/resident rights.
• Provides education to patients/families/caregivers as directed.
• Conducts self in a professional manner per policies.
• Works rotating shifts, holidays and weekends.
• Initiates emergency support measures (i.e., CPR).

LPN (Licensed Practical Nurse)

Job Description

• Works under direct supervision using the Nurse Practice Act, Company standards, Policies and Procedures and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Assesses patients/residents and makes observations and report pertinent information related to the care of the patient/resident.
• Implements the patient/resident plan of care and evaluates the patient/resident response.
• Directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e. monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident personnel information.
• Provides patient/resident/family/caregiver education as directed.
• Conducts self in a professional manner in compliance with unit and facility polices.
• Works rotating shifts, holidays and weekends as scheduled.
• Initiates emergency support measure (i.e., CPR, protects patient/residents from injury).

LPN (Licensed Practical Nurse)

Qualifications/Requirements

• Current/valid Florida LPN (Licensed Practical Nurse) License
• Previous experience in long term care preferred.
• Current CPR card
• Ability to work independently or part of a group.
• Effectively communicate with others.
• A minimum of one (1) year nursing experience in a long-term or acute care setting preferred.

Shifts Available: Full Time and Part Time
