Job summary

Interim HealthCare of Northern Indiana is seeking an Home Health Licensed Practical Nurse (LPN) in Merrillville, IN to deliver one-on-one in-home care. You will work with an interdisciplinary team to provide high-quality care to patients after hospital discharge and manage ongoing home health needs.

Qualifications include graduation from an accredited LPN program, active Indiana LPN license and CPR certification. Experience in home health and knowledge of relevant regulations are preferred.

Qualifications

• Graduate of an accredited practical nursing program.
• Active LPN license in Indiana required.
• CPR certification required.
• Knowledge of home health regulations preferred.

Responsibilities

• Provide home-based care to patients in their homes.
• Take patient vitals, perform assessments, document progress.
• Assist RN in carrying out patient’s plan of care and teach self-care.
• Educate family on plan of care, medications and home safety.
• Collaborate with interdisciplinary team to coordinate care and discharge planning.
