Job summary

Oak Grove Manor Health Care Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our dedicated team for full-time and part-time night shifts (6pm-6am). You will conduct assessments, manage admissions and discharges, provide wound care, and lead CNAs and CMTs while upholding facility protocols and responding to emergencies.

We value genuine empathy and patience in caring for residents. LPN license is required.

Qualifications

• Must have a valid LPN license.
• Demonstrates empathy and patience when caring for residents.
• Experience in long-term care or similar setting preferred.

Responsibilities

• Conduct assessments, follow protocols, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
