Qualifications

• Current Colorado Licensed Practical Nurse (LPN) license or compact state license.
• Graduate of an accredited practical nursing program.
• Strong communication and clinical skills.
• Compassionate, dependable, and team-oriented.
• Long-term care experience is preferred but not required.
• Passion for providing exceptional resident-centered care.

Responsibilities

• Provide compassionate, resident-centered nursing care.
• Administer medications and treatments in accordance with physician orders and your scope of practice.
• Monitor resident conditions and promptly communicate changes to the RN, physician, or DON.
• Collaborate with physicians, RNs, therapists, and other healthcare professionals to ensure quality resident outcomes.
• Maintain accurate and timely clinical documentation.
• Promote resident safety, dignity, comfort, and independence.
• Support and mentor CNAs.
• Ensure compliance with state and federal regulations, facility policies, and nursing standards.
• Contribute to a positive, team-focused culture centered on exceptional resident care.

What You'll Do

As a Licensed Practical Nurse, you'll play an important role in delivering exceptional resident care while working alongside a dedicated interdisciplinary team.

Responsibilities include:

• Provide compassionate, resident-centered nursing care

• Administer medications and treatments in accordance with physician orders and your scope of practice

• Monitor resident conditions and promptly communicate changes to the Registered Nurse, physician, or Director of Nursing

• Collaborate with physicians, Registered Nurses, therapists, and other healthcare professionals to ensure quality resident outcomes

• Maintain accurate and timely clinical documentation

• Promote resident safety, dignity, comfort, and independence

• Support and mentor Certified Nursing Assistants (CNAs)

• Ensure compliance with state and federal regulations, facility policies, and nursing standards

• Contribute to a positive, team-focused culture centered on exceptional resident care
