Qualifications

• Must hold a current LPN license.
• Previous experience as an LPN is preferred.

Responsibilities

• Provide direct patient care to residents, including medication administration, wound care, and vital sign monitoring.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Maintain accurate and up-to-date records of patient care and treatment.
• Report changes or concerns to the management team.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
