Shawnee-Christian-Healthcare in Louisville, KY seeks an Licensed Practical Nurse to provide professional nursing care under supervision of physicians and nurse practitioners. The role involves direct patient care, vital signs, medication administration, and supporting providers in exams and care coordination.

The ideal candidate will have a Kentucky LPN license, CPR/BLS, and at least two years in a medical clinic.

Qualifications

  • Graduate of an accredited school of practical nursing.
  • Current Kentucky LPN license.
  • CPR/BLS certification required.
  • Minimum two years of professional nursing experience, preferably in a medical clinic setting.
  • Knowledge of examination, diagnostic and treatment room procedures.
  • Skill in assisting in a variety of treatments and medications as directed.
  • Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public.
  • Knowledge of MS Word, Excel, Outlook.

Responsibilities

  • Support patients in navigating healthcare services and community resources.
  • Utilize EHR reports and population health tools to support quality metrics.
  • Prepare exam rooms and instruments; maintain specimen handling and testing.
  • Attend required meetings, in-services, and daily huddles.

The Licensed Practical Nurse (LPN) is responsible for providing professional nursing care to patients under direction of Nurse Practitioners and MDs.

Essential Duties and Responsibilities
  • Escorts patient to exam room and assists in determining medical problems
  • Greets patients in a courteous and friendly manner
  • Takes vital signs and assists in patient flow through the clinic - Vital signs: blood pressure, temperature, height, weight, and BMI
  • Administers prescribed medications and treatments in accordance with nursing standards
  • Assists in performing Comprehensive Health Assessment: documents immunizations, family/social/cultural characteristics, communication needs, patient/family/medical/mental health/surgical history, advanced care planning, behaviors affecting health, depression screening
  • Assists the providers with patient education and helping ensure quality of care for patients (i.e. assuring patients are receiving their preventative screenings timely).
  • Assist in identifying and addressing patient care gaps related to preventive care, chronic disease management, screenings, immunizations, and follow-up appointments.
  • Conduct patient outreach for overdue services, abnormal results, referrals, and care coordination needs.
  • Support patients in navigating healthcare services, appointments, referrals, medications, and community resources.
  • Collaborate with providers, referral coordinators, behavioral health staff, clinical pharmacists, and community health workers to improve continuity of care
  • Assist with transitions of care and follow-up for high-risk or medically complex patients.
  • Utilize EHR reports, registries, and population health tools to support quality metrics and value-based care initiatives.
  • Responds to and refers incoming calls to the provider as deemed appropriate
  • Administers medications/give injections at direction of provider.
  • Participates in Plan-Do-Study-Act cycles to improve performance on targeted metrics
  • Prepares exam rooms and instruments for examination and testing
  • Labels and maintains proper storage for specimens and tests. Delivers tests and obtains results
  • Ensures that lab reports, x-rays, and other information are in patient charts
  • Assists providers with minor diagnostic procedures and treatments as directed
  • Issues outpatient materials. Assists in application and patient instruction
  • Attends required meetings, in-services, and daily huddles
  • Track and order supplies for the medical clinic
  • Oversee the workflow of patient care gaps

Job Qualifications and Requirements

Working Environment and Physical Requirements

  • Work is performed in clinical area and involves contact with patients in office/ via phone
  • Work may be stressful at times. Interaction with others is constant and interruptive. Contact involves dealing with sick persons.
  • Requires full range of body motion including manual and finger dexterity and eye-hand coordination. Requires standing and walking for extended periods of time. Occasionally lifts and carries items weighing up to 30 pounds.
  • Requires normal visual acuity and hearing
  • Requires working under stressful conditions or working irregular hours
  • Local travel may be required to attend meetings or visit other SCHC clinics
