Job summary

Elara Caring is hiring a Home Health LPN to serve patients across Lorain County and western Cuyahoga County. Join a team delivering one-on-one nursing care in the home setting, building strong patient relationships and ensuring safe, effective care in patients' homes.

You will follow individualized care plans, monitor progress, and collaborate with the RN Case Manager to adjust plans as needs change. A compassionate, reliable nurse with driving capability is required.

Qualifications

• Current state license as an LPN.
• Graduate of an accredited nursing program (certificate, diploma, or associate).
• At least 1 year of clinical care experience.
• Ability to lift up to 50 pounds and to travel between patient homes.
