Job summary

SWLA Center for Health Services is hiring a Licensed Practical Nurse (LPN) in the Nursing Support department. The role supports multidisciplinary care, greets patients, collects information for physicians, and assists with procedures, medications, and labs under physician direction.

Candidates must have a practical nursing credential, valid Louisiana licensure if applicable, and current BLS/IV certification.

Qualifications

  • Must have Practical Nursing diploma or certificate from an accredited program.
  • Must meet Louisiana Board of Practical Nursing requirements and hold valid licensure if applicable.
  • Current BLS/IV certification and CEUs or equivalent for re-licensure.
  • Maintain professional attitude under pressure and write legibly for paperwork.
  • Demonstrate proper English grammar in documentation and professional image for community relations.

Responsibilities

  • Greet patients and minimize waiting time in the exam room.
  • Collect patient information to assist physician in care.
  • Assist physician with procedures; administer medications within scope; draw labs.
  • Interpret physician instructions to patient and family.
  • Coordinate follow-up care and reschedule no-shows.
  • Prepare and maintain exam rooms, equipment, supplies and environment.
  • Participate in training and health promotion activities as part of the health team.
  • Perform additional duties as prescribed by Nursing leadership.

JOB DESCRIPTION
JOB TITLE: LPN
DEPARTMENT: Nursing Support
SUPERVISED BY: Chief Nursing Officer
SUMMARY

In keeping with standards recognized by both NCQA Medical Home Recognition and JACHO, SWLA has created teams of professionals to care for the clinical needs of patients who choose SWLA as their primary care provider. The team consists of physicians, mid-level practitioners and the nursing support which is described in this job description. Given the nature of the duties required by the nursing support personnel, SWLA will recruit and hire applicable personnel in the categories of Medical Assistant, Certified and non-certified, and Licensed Practical Nurses. Salary will be adjusted for education, licensing acquisition and experience.

EDUCATION, TRAINING AND EXPERIENCE
  1. High School Graduate. Successful completion of Practical Nursing course from an accredited Technical School.
  2. Successful completion of all requirements of Louisiana Board of Practical Nursing must be met if applicable. Current State Licensure if applicable.
  3. Current BLS/IV Certification. CEU'S or equivalent contact hours for re-licensure or re-certification, if applicable. Must successfully complete a competency exam.
  4. Must maintain a professional attitude under pressure.
  5. Employ correct grammatical English and write legibly in order to complete appropriate paper work.
  6. Present a professional image and be able to maintain the highest level of quality patient and community relations in representing the foundation.
JOB RESPONSIBILITIES
  1. Participates as an integral part of the multidisciplinary care team.
  2. Responsible to greet patients and ensure that they do not spend excess waiting time in the exam room.
  3. Responsible for the collection of necessary information concerning the patient in order to assist the physician in providing medical care. Necessary information is inclusive of Chief Complaint, any specific physical or clinical complaints that may necessitate cursory evaluation of the patient.
  4. Ensure that patients are properly disrobed for the type of visit that they are being evaluated for.
  5. Assist the physician with necessary procedures and examinations, gives medications within scope of practice as ordered by the physician, draws laboratory specimens and carries our assigned duties in providing total patient care.
  6. Responsible for the interpretation of physician’s instructions to the patient and/or family to meet immediate health needs.
  7. Responsible for the arrangements for follow-up care when required for patients’ needs.
  8. Responsible for calling the patients who have "no-showed" their visit, to reschedule their appointments.
  9. Responsible for the preparation and maintenance of exam rooms, clinic equipment, supplies and environment for general operations.
  10. Participates in training and health promotion activities as a member of the health team.
  11. Performs additional duties as prescribed by the Director of Nursing, Chief Medical Officer and assigned Medical Provider.
  12. Assumes additional duties when necessary to ensure quality health services are being rendered by clinic.
