Job summary

Bridgeway seeks a Nurse Clinician to provide outpatient nursing services at our Kewanee, IL site. You will work with a team to deliver medication monitoring, case management, and wellness interventions according to individual treatment plans.

The role of the Nurse Clinician is responsible for providing outpatient nursing services using a variety of therapeutic modalities.

• Provide for informed consent regarding medication purpose, side effects and adverse reactions including tardive dyskinesia to Medication Clinic consumers- renewing this consent on an annual basis. Assist in the coordination and maintenance of a tardive dyskinesia monitoring system for consumers in accordance with Rule 132 and CARF standards.
• Provide case management services, medication monitoring and medication training to mentally ill consumers in the program, Ensure the clinical course of each case follows the treatment plan and that this is well documented in the medical record. Work well with all departments of Bridgeway and is able to resolve problems in a productive manner.
• Assess medication Clinic consumers' knowledge of medication and develop goals with consumers in regard to achieving knowledge of his/her mental illness, medication effects and side effects, self-administration, storage and safeguarding of medication.
• Ensure comprehensive health care services by monitoring consumers physical condition, providing nursing assessment as needed and ensuring linkage to case management and community health care providers. Monitor consumers mental condition, observing for early signs of decompensating and providing the necessary mental health interventions to promote wellness.
• Assist in operation of the medication clinic in conjunction with psychiatric services. This includes client assessment, administration of long-acting injectable neuroleptic, maintaining supplies and med clinic records, including the development, maintenance and regular review of medication clinic policy.
• Assist with implementation of Clozaril management system in conjunction with psychiatric services following established protocols.

Minimum requirements include an Associate's Degree in an accredited nursing program. Must maintain a valid nursing license in Illinois. Prior experience with elderly clients and one year of mental health experience is preferred. Must have reliable transportation, valid driver's license, and meets organization's insurability requirements. Employment with Bridgeway is conditional upon completion of all background checks.
