Job summary

Signature HealthCARE of Spencer County is seeking an LPN/LVN to provide direct nursing care, administer medications, and support residents with daily activities in a 120-bed skilled nursing setting. You will supervise nursing assistants, coordinate with the interdisciplinary team, document care plans and outcomes, and participate in quality improvement activities to ensure compassionate, compliant care.

Active Kentucky LPN/LVN license and CPR/BLS certification preferred; 1 year of clinical

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred; American Heart Association or American Red Cross.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift per facility policies and regulations.
• Monitor and observe residents' physical and emotional status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

How you Will make a difference

• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
