Job summary

Maplewood Health Care Center is seeking motivated LPNs to join our care team in Jackson, Tennessee. This full-time position supports our residents with compassionate, team-driven care.

As an LPN, you will supervise CNAs, manage admissions and transfers, administer medications, and maintain care plans while upholding patient confidentiality. Tennessee LPN license and CPR/BLS certification are required.

Qualifications

• Current unrestricted LPN License for the state of Tennessee.
• CPR/BLS Certification required.

Responsibilities

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess residents' conditions, prepare and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with work.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
