Job summary

PowerBack Rehabilitation in West Virginia is seeking a Licensed Practical Nurse (LPN) to provide patient-centered nursing care under RN supervision at our long-term care facility. You will support assessments, care planning, and medication administration in a team-focused environment.

You will evaluate patient status, document changes, and supervise unlicensed staff while maintaining high-quality, compassionate care for residents.

Qualifications

• Graduate of an approved practical nursing program (LPN).
• Current-state LPN licensure or pending licensure.
• Current BLS/CPR certification as required by practice setting.

Responsibilities

• Deliver nursing care under RN direction and document outcomes.
• Evaluate patients to observe condition and report changes.
• Contribute to nursing assessments and care plans.

Responsibilities

• Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse delivers efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction.
• Evaluate patients/residents to collect data, observe condition and report changes in condition.
• Contribute to nursing assessments and care planning.
• Administer medications and performs treatments per physician orders.
• Supervise and monitor patient care provided by unlicensed staff.

Qualifications

• Must be a graduate of an approved school for practical or vocational nursing.
• Must be currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.
• Must maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.
