Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility. You will deliver total nursing care to residents on the assigned unit and participate in individualized care planning with the healthcare team.

Ideal candidates hold an active LPN license in Illinois, demonstrate strong communication and teamwork, and are committed to quality resident care while adhering to facility policies and safety standards.

Qualifications

• Graduation from an accredited nursing education program.
• Active LPN license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies and maintain safety.
• Ability to work with diverse resident populations, including elderly and mentally ill individuals.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as a Licensed Practical Nurse (LPN).
• Participate in developing and implementing individualized care plans alongside the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness regularly.
• Maintain an environment that promotes residents’ physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action if needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
