Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility in Glenwood, IL. You will provide total nursing care to residents, supervise team members, and ensure compliance with facility standards.

You will develop individualized care plans with the healthcare team, administer medications, and educate residents and families on care plans. Strong communication and safety adherence are essential.

Qualifications

• Must graduate from an accredited nursing education program.
• Active LPN license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an LPN.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness regularly.
• Maintain an environment that promotes residents’ physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development and evaluation.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
• Follow all facility safety policies and procedures.
