Job summary

Hadley Pointe for Nursing Rehab and Care in Hadley, MA is seeking a compassionate Licensed Practical Nurse (LPN) to join our team on a full-time basis. You will provide high-quality nursing care to residents, including medications, wound care, and chronic disease management.

You will collaborate with an interdisciplinary team to develop individualized care plans and monitor residents’ conditions, reporting changes and recommending interventions as needed, while supporting resident independence

Qualifications
