Job summary

Hadley Pointe for Nursing Rehab and Care in Hadley, MA is seeking a compassionate Licensed Practical Nurse (LPN) to join our team on a full-time basis. You will provide high-quality nursing care to residents, including medications, wound care, and chronic disease management.

Qualifications

• Massachusetts LPN license required.
• High school diploma or equivalent; associate's or bachelor's degree preferred.

Responsibilities

• Provide nursing care including administering medications, wound care, and chronic disease management.
• Develop and implement individualized care plans with the interdisciplinary team.
• Monitor and report changes in resident conditions and recommend interventions and treatments.
• Perform vital signs, inventory medications, and maintain accurate records.
• Collaborate with rehabilitation team to promote resident independence and well-being.
• Participate in ongoing education and training to stay current with industry standards and best practices.
• Support a positive, respectful, and inclusive environment for all residents, staff, and visitors.
