Job summary

Mountain City Rehab Center in Frostburg, Maryland is seeking a dedicated Licensed Practical Nurse (LPN) to join our 99-bed skilled nursing facility. You will provide direct, high-quality care to residents under the supervision of the Nurse Manager and DON, working closely with an interdisciplinary team to achieve optimum outcomes.

The ideal candidate holds a Maryland LPN license, CPR/BLS, and strong clinical skills. Prior long-term care experience is preferred but not required.

Qualifications

• Must hold a current Maryland LPN license.
• CPR/BLS certification or willingness to obtain on hire.
• Experience in long-term care is preferred but not required.
• Strong clinical skills and patient-focused care.

Responsibilities

• Provide direct nursing care, including administering medications, treatments, and monitoring vital signs.
• Assist with development, implementation, and evaluation of individualized care plans.
• Document resident status, treatments, and responses in accordance with facility policy and state/federal regulations.
• Monitor residents for changes in condition and report promptly to the RN or physician.
• Collaborate with interdisciplinary team members, residents, and families to ensure optimal resident outcomes.
• Supervise and support GNAs in the delivery of care.
• Maintain a clean, safe, and therapeutic environment for residents.
• Ensure compliance with infection control, safety, and HIPAA policies.
• Participate in quality assurance activities and staff meetings.
• Fulfills the role of charge nurse when assigned.

Requirements

• Current and valid LPN license in the state of Maryland or Multistate
• Graduation from an accredited Practical Nursing program
• CPR/BLS certification (or willing to obtain upon hire)
• Prior experience in a long-term care or skilled nursing facility preferred but not required
• Strong clinical skills and attention to detail
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment
