About the Role:

As a Per Diem LPN, you will be responsible for providing high-quality, individualized care to our patients on a day shift schedule. You will work closely with our interdisciplinary team to develop and implement patient care plans, assess patient needs, and administer medications and treatments. Your excellent communication skills will be essential in collaborating with patients, families, and healthcare professionals to ensure seamless care delivery.

Responsibilities:

• Provide skilled nursing care to patients, including assessing patient needs, developing and implementing patient care plans, and administering medications and treatments.

• Collaborate with the interdisciplinary team to develop and implement patient care plans, ensuring continuity of care and coordination of services.

• Monitor patient progress, documenting and reporting any changes or concerns to the healthcare team.

• Administer medications and treatments, including injections, wound care, and vital sign monitoring.

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery.
