Mill Brook Rehabilitation & Healthcare Center in Fall River, MA seeks a Licensed Practical Nurse (LPN) for full-time and part-time roles. The position offers night and evening shifts with competitive hourly pay and a $3,000 sign-on for full-time.

Responsibilities

• Provide patient care per the plan of care and physician orders.
• Follow individualized care plans created by the Nurse Manager and team.
• Administer medications and prescribed treatments per policy.
• Monitor residents, report changes promptly, and respond to needs.
• Maintain accurate documentation in line with orders and standards.
• Support a safe, respectful environment for residents and families.

$35-$44/hour (all inclusive)
Full-time: 11 PM - 7 AM

Part-time: 3 PM - 11 PM

$3,000 Sign-On bonus for Full-Time

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of MA.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
