Job summary

Sage Specialty Hospital (LTAC) in Denham Springs, LA is seeking full-time LPNs for day shift. Responsibilities

• Provide nursing and supportive care per the Interdisciplinary Plan of Care.
• Coordinate with attending physicians and hospice team to ensure optimal patient care.
• Document all services provided and submit on required forms.
• Demonstrate knowledge of Medicare/Medicaid hospice regulations.
• Report changes in patient condition to the RN for care plan updates.
