Qualifications

• Associate degree or equivalent nursing experience.
• Minimum of one year nursing experience with individuals with disabilities.
• Valid RN license.
• Valid driver’s license with satisfactory driving record.
• Ability to calculate medication dosages.
• Commitment to community-based support for persons with disabilities.
• Ability to stay calm in crisis situations.
• Strong verbal and written communication skills.

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by standards of practice or law.
• Ensure individuals attend medical appointments per their ISP.
• Advise supervisor of concerns related to nursing services.
• Create and distribute medical/health reports with health goals and physician recommendations.
• Conduct trainings including medication administration and delegation.
• Monitor health conditions and refer for medical services as needed.
• Train employees on medical needs of individuals.
• Maintain medication administration logs and health service supplies.

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 11 states.

We are in search of a Licensed Practical Nurse in Cookeville, TNto deliver care to individuals with disabilities in a community setting, utilizing the nursing process of assessment, planning intervention, implementation, and evaluation. The Registered Nurse delivers care to individuals with disabilities in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.

Starting pay is $25.00/ per hour

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
• Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
• Advise supervisor of all concerns and issues related to the provision of nursing services
• Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
• Conduct programmatic trainings including medication administration and delegation
• Conduct site visits, complete medication audits, and related monitoring activities
• Train employees on the medical needs of assigned individuals
• Monitor health and medical conditions of individuals and refer for medical services as needed
• Monitor the use of medications through medication administration logs
• Ensure health service supplies are available and replenished as needed
• Ensure program documentation and billable records are completed accurately and timely
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
• Participate with individuals in community, social, and recreational activities
• Participate in circles of support for personal futures planning
• Follow strict OSHA procedures regarding blood borne pathogens
• Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation

Minimum Requirements

• Associate degree or equivalent experience in nursing field
• Minimum of one year of experience providing nursing services to individuals with disabilities
• Valid R.N. license
• Valid driver’s license with a satisfactory driving record
• Complete all state and agency required training per state guidelines
• Ability to calculate figures related to medication dosages
• Understanding and commitment to community-based support for persons with disabilities
• Ability to exercise good judgment and remain calm in crisis situations
• Ability to establish a comfortable and supportive relationship with an individual receiving supports
• Excellent verbal and written communication skills
• Ability to effectively present facts and recommendations
• Ability to effectively communicate with program participants, third-party agencies, staff and members of the community
